This helps us understand how to support you safely, respectfully, and in line with your goals.

You are partnered with a designated key worker who works alongside you to co-create a personalised support plan.
Your plan is built around:

• your strengths
• your goals and outcomes
• the level of support you want

We make sure the plan feels right for you, clear, practical, and built around the life you want to live.

We carefully match you with carers whose skills and approach align with your needs and preferences.
This helps build trust, consistency, and a positive working relationship from the start.

Where possible, we introduce you to your carers before support begins so you feel comfortable and confident.
